How to make a claim

Should your eyewear require adjustment or cleaning please visit your preferred store for assistance.

Eyewear Protection Insurance covers accidental damage and theft only a Co-Payment is payable for all insurance repairs or replacements.

To make a claim please choose from one of the methods below.
You will need the following information when making a claim:

• Policy Number / Transaction Number.
  If you are unsure, you can obtain this from your original purchase docket.
• If your claim is a result of Theft you will need the police report number.
